The Medical Social Worker renders professional psycho-emotional, social & spiritual care and support to Dover Park Hospice clients and in doing so, help enhance their quality and of life and thus leading more meaningful lives. He/She also plays an active role in the planning and execution of the activities of the Hospice that promotes the objectives and the mission of the Hospice
Responsibilities
Clinical Assessment & Intervention
- Conduct comprehensive bio-psychosocial-spiritual assessments to evaluate patients' and families' physical, emotional, social, financial, spiritual, and environmental needs
- Perform ongoing follow-ups and formulates intervention care plans based on patient and family needs
- Provide counselling to patients and families on issues related to loss, grief, death, and dying
- Assess risks related to self-harm, harm to others, or neglect, intervening as needed to ensure safety and well-being
Case Management & Coordination
- Conduct financial assessments to ensure care affordability and optimizes available financial resources
- Facilitate communication between patients, families, and the clinical team to promote informed decision-making
- Coordinate referrals to community resources, support services, and financial assistance programs to enhance patient care access
- Assist with discharge planning and transitional care to ensure continuity between healthcare settings
- Maintain accurate and timely case notes and documentation for assigned caseload
Team Collaboration & Supervision
- Communicate effectively with the multi-disciplinary team to address psychosocial issues and coordinate care
- Participate in multi-disciplinary rounds and supports patient care decision-making
- Assist in the orientation, training, and supervision of team members, including Social Work Interns
Community Engagement & Program Development
- Develop and conduct therapeutic group work, such as support groups, for clients
- Assist in environmental scanning and community needs assessments through data collection and analysis
- Collaborate with other agencies to pool community resources and improve care access
Professional Development & Advocacy
- Promote the practice of palliative care by participating in research and engaging in public speaking to elevate the social work profession
- Conduct training for the team and stakeholders to enhance skills and knowledge
- Prepare for job rotation across different programs (Inpatient, Home Care, and Day Care) to optimize manpower and ensure professional growth
Other Duties
- Assist in the organization of the memorial event and any other department projects assigned by the HOD and/or management
- Provide reports on work done to the HOD and undertakes projects as assigned by HOD and/or Supervisor
- Report on Social Work and Psychosocial services for publications by Dover Park Hospice and/or the palliative fraternity
Requirements
- Bachelor Degree in Social Work or Graduate Diploma in Social Work from a recognised institution
- Accredited Social Worker with Social Work Association of Singapore
- 4 years of clinical experience
- Possesses good interpersonal and communication skills as well as problem-solving skills
- Energetic, committed and enjoy working as a team
The successful candidate can expect a monthly basic salary up to $5,300, commensurate with experience.
