The Social Work Associate supports the Medical Social Workers and Expressive Therapists in providing holistic care to patients and families, assisting with administrative tasks, assessments, and intervention plans. Offers psycho-emotional, social, and spiritual support to enhance the quality of life for DPH clients. Actively contributes to planning and executing hospice activities that align with its mission and objectives.
Responsibilities
A. Care Coordination & Administrative Support
- Assist the clinical team with patient referrals for care and financial support (e.g. Home-Based Respite Care, Home Personal Care).
- Liaise with internal and external service providers to ensure timely and appropriate support for patients.
- Identify and maintain a directory of relevant community and financial resources.
- Support coordination of community services (e.g. HRHS List).
B. Case Management Support
- Conduct initial intake assessments to identify patient needs, strengths, and risks.
- Maintain accurate and timely case documentation and notes.
- Perform financial assessments and provide basic counselling to ensure affordability and access to resources.
- Support implementation of care plans for stable patients under the guidance of Medical Social Workers or Supervisor.
- Monitor patient progress and provide regular updates to the care team.
- Maintain contact with patients and families to provide ongoing support.
- Participate in multidisciplinary rounds.
C. Group Work & Patient Engagement
- Assist in planning and preparation of patient and caregiver engagement activities (e.g. caregiver support sessions, home care outings).
D. Departmental Administrative Support
- Record meeting minutes and track follow-up actions.
- Maintain and update department resource folders and active patient lists.
- Coordinate data collection and reporting to ensure accuracy and accessibility.
- Support research data collection and assist in departmental reports as needed.
E. Other Duties
- Assist in organising departmental and memorial events.
- Prepare work reports and contribute to internal and external publications.
- Undertake projects and tasks as assigned by the Supervisor, HOD, or management.
Requirements
- Diploma in Social Sciences (Social Work) or Social Services with a recognised institution
- 1 – 2 years of relevant experience in a healthcare environment and a total of at least 0 – 5 years of clinical experience
- Empathetic, self-motivated team player with strong communication and interpersonal skills.
- Competent in MS Office applications especially in MS Excel
The successful candidate can expect a monthly basic salary up to $3,600, commensurate with experience.
